Case Management
Community Support Services
This program offers individualized psycho-social supports with a mental health worker, in a safe, confidential atmosphere, in which the person is encouraged to establish personal goals and action plans which is tailored to meet the unique needs of the individual. Some aspects would include: community outreach, assessment/referral, crisis prevention and intervention, life and social skills development, stress management, education/coping strategies, symptom and medication management, leisure skills, linkage and advocating, coordinating services with other agencies, health care workers and with resources in the community i.e. Ontario Works and Ontario Disability Support Program.
INTAKE INFORMATION AND REFERRAL
CMHA has a centralized intake service. There is one person accepting all referrals for the Community Support program and Supportive Housing. The intake worker is also provides information about CMHA’s services and other community mental health resources. This service will make referrals to community agencies and be available to answer general questions about mental health and mental illness. The intake worker remains the contact for individuals on the waiting list. Monthly group meetings are provided to those on the waiting list who wish to receive peer support while waiting to receive Community Support.
VOLUNTARY FINANCIAL PLANNING AND MONITORING
This program is designed to establish a formal voluntary financial agreement with clients to assist in achieving their financial management goals.
SOCIAL RECREATION PROGRAM
Adults experiencing mental health difficulties may benefit from participating in the wide range of social, recreational and vocational opportunities offered through our “Friends In Motion” Social Recreation Program. Focus is on providing recreation leisure opportunities to improve social skills, quality of life and enjoyment.
Participation in the program activities assists members to develop and build their self-confidence, learn a variety of life skills, increase socialization activities, strengthen informal support contacts, foster peer support and belong to a supportive community setting.

SUPPORT GROUPS AND WORKSHOPS
Our self help groups are offered at various times throughout the year. They are facilitated educational programs with a goal to create an environment based on mutual support, empathy and continuation of a developed network of support.

EARLY PSYCHOSIS INTERVENTION (LYNX PROGRAM)
The program provides early intervention for people 14 to 45 experiencing a first episode of psychosis. This is a treatable brain disorder characterized by changes in thought, perception, emotion and behavior, often emerging during adolescence and young adulthood. With prompt attention and the involvement of family in the treatment process, most people can make a good recovery.
FAMILY CAREGIVER NETWORK
This is a grass roots, self help, support, education and advocacy organization for family members and caregivers of those with serious mental illness. Serving the City of Kawartha Lakes, Haliburton, Northumberland and Peterborough Counties, and helping you connect with professional mental health service providers, community based supports and free workshops.
